In older boys and men, circumcision may be necessary when the foreskin becomes too tight to be pulled back comfortably over the tip of the penis (phimosis). At West Coast Urology, each patient is evaluated and specifically offered the appropriate treatment based on type and severity of his symptoms. Treatments may include instruction on daily hygiene, application of topical creams, frenulotomy (to divide a tight band that causes bending down of the tip of the penis), dorsal slit (an incision in the foreskin to allow to pull it back in an emergency), and circumcision. Other conditions which may necessitate a circumcision include recurrent infections or the occurrence of lesions or tumors of the foreskin.
